    Are the thoughts true? You get them because you are human and the way that the mind works. Everyone, including me, gets random thoughts every day, all days.

    You can check this out for yourself; just sit back and let your mind do its thing for the next minute. Sometimes it is hard to catch all the thoughts that stream through your mind, and some thoughts stick out. The random thoughts that pop into your head, or my head, are involuntary; you have no control over this, as you will have seen if you took a minute and let your mind wander, as I suggested above.

    How you react to the thoughts, though, this you do have control over, is one of the differences between people who hardly notice these thoughts and people who are struggling because of their thoughts. Do they say anything about me as a person?

    No, they say that you are struggling with unwelcome thoughts; they do not reflect you as a person; it is just that they feel real.

    The only thing that says something about you as a person is how you choose to live your life. The real stuff you do in the real world. People who know me or read my stuff on the website assume I am a good person, and I agree with that. I judge myself on how I carry out my life, not by the thoughts in my head, as I know that I am just as capable as the next person to have thoughts in my head that do not reflect me as a person.

    Why do my HOCD thoughts feel real? If your brain sees thoughts relating to your sexual identity as a threat, it will stand up and take notice. You can see what I mean by a threat in the video. It is a recording of a webinar on panic attacks, but it explains threats clearly and will hopefully help you understand what I mean. Hopefully from what you have already read you will know the answer to this is no.

    HOCD related thoughts are just thoughts. It is just a thought that I am capable of thinking. You might think they are different because they disturb you, are so out of character and are stuck in your head all the time, but that still does not make them say something about you as a person. They create a nagging doubt in your mind. Once you gain control, the emotion starts to fade. When the emotion has gone, they do not feel real; the thoughts have lost their grip on you.     I include Acceptance and Commitment Therapy to help with HOCD and this, along with other techniques, helps to take the emotion and fear away from the thoughts.

    Child sexual abuse is the most hated crime one can commit in society, one that is taboo in all cultures. Even among murderers and other felons in the prison system, the child sexual predator is the pariah of the group, and for good reason. I use the word pedophile more than the general population, so much so that I have habituated to using the word.

    POCD sufferers are often very relieved to observe my non-reaction to pedophilia-themed thoughts when they finally become brave enough to share them. OCD thoughts can be as individualized as your fingerprint. An individual with POCD often has uncertainty about whether his situation meets the criteria for OCD, fearing this answer may render him a pedophile.

    Even if one realizes POCD exists for some people, there is fear that he is the exception to the rule and in this case it is real pedophilia bubbling under the surface. It makes one question his very being, and ability to feel like a good and moral person.

    With the amount of suffering that is taking place, individuals with POCD look anywhere for certainty and relief. Compulsions are behaviors that are performed in response to obsessions that are used in an attempt to gain certainty about being a pedophile or to seek comfort from anxiety and guilt for having such decidedly taboo thoughts.

    Here are some compulsions related to POCD: Avoid normal physical contact with children such as hugs and hand holding Avoid giving children baths or changing clothing or diapers Avoid watching movies, television shows with pedophilia themes Avoid reading books or articles with pedophilia themes Internet research to determine if one fits the mold for pedophilia Asking for reassurance from trusted loved ones about fears Checking for physical arousal around children, sometimes resulting in unwanted sensation Mentally reviewing past events for indicators of pedophilic behavior Mental analysis of sexual orientation and self-reassurance about a preference for adults Individuals with POCD are usually very afraid to speak to a therapist for fear he or she will be misdiagnosed as a pedophile.

    They may even have had bad experiences where untrained mental health professionals suggest they were abused as children, that the intrusive thoughts mean something deep about who they are as people, or worse.

    For this reason, individuals with POCD often suffer in silence for months or years before getting help. Despite being a common OCD theme I see regularly in my practice, it is not a well-known theme to the general community. Even clients who have successfully treated their POCD only usually feel comfortable sharing their theme with other sufferers, close family or their mental health team. For this reason, public awareness has been slower than for other forms of OCD.

    It is also invisible. You may know people suffering with POCD and not even know it. Successful treatment for POCD is available and there is a lot of hope with the right kind of therapy. The gist of therapy for OCD is to recognize how OCD has hijacked the brain and how it distorts thinking to trick the sufferer into feeding it with compulsions.

    Once the assessment is complete, your therapist will then educate you on the next steps. They will explain the treatment plan and will also educate you on tools that will help you manage your obsessions, such as mindfulness tools, self-compassion tools and acceptance and commitment therapy ACT. ERP for pedophilia obsessions consists of exposing oneself to the feared stimuli--in this case, being around children--and then learning to not engage in compulsions.

    ERP is a tiered system created together by the therapist and client, starting off slow and working up to eliminating as many compulsions as you can. The key to recovery of any type of OCD is learning to accept uncertainty and tolerate discomfort. You allow your thoughts to come and go without automatically attaching meaning or truth to them.     For this reason, public awareness has been slower than for other forms of OCD. ERP exposes patients to situations related to their intrusive thoughts that cause them anxiety. The goal of this treatment is for the patient to prevent himself or herself from completing their compulsive behaviors when triggered by intrusive thoughts. The situations that are confronted will intensify over time, until the patient can face and overcome their most feared scenario.

    Once they are able to stop themselves from responding to their intrusive thoughts with compulsive behaviors, they can experience tremendous relief from the symptoms of OCD.
